Clarity Therapy needs to collect information about you for the primary purpose of providing a quality service to you. In order to thoroughly assess and provide therapy, we need to collect some personal information from you. If you do not provide this information; we may be unable to treat you. This information will also be used for:

a. The administrative purpose of running the practice;

b. Billing either directly or through an insurer or compensation agency;

c. Use within the practice if discussing or passing your case to another practitioner within the practice for your ongoing management;

d. Disclosure of information to your doctors, other health professionals or to teachers to facilitate communication and best possible care for you; and

e. In the case of insurance or compensation claim it may be necessary to disclose and/or collect information that concerns your return to work to an insurer or your employer.

We do not disclose your personal information to overseas recipients.

Clarity Therapy has a Privacy Policy that is available on request and is available on their website. That policy provides guidelines on the collection, use, disclosure and security of your information. The Privacy Policy contains information on how you may request access to, and correction of, your personal information and how you may complain about a breach of your privacy and how we will deal with such a complaint.

To ensure the process of quality treatment provision, information about your assessment results and progress may be given to relevant other service providers, who are involved in your management. These may include your doctor, teachers, specialists, insurers, solicitors or employers.

National Disability Insurance Scheme (NDIS)

If [Patient First Name] [Patient Surname] is accessing NDIS services with Clarity Therapy, this consent form serves as a formal Service Agreement between [Patient First Name] [Patient Surname] and Clarity Therapy Pty Ltd. The number of sessions you require are to be agreed upon with Clarity Therapy in conjunction with the client and/or their support coordinator or participant nominee. The fee for our services falls within the below item outlined in the March, 2021 NDIS Price Guide: $193.99 (15_056_0128_1_3) per 50 minute client session. Please inform your Plan Manager this rate is for Assessment, Recommendation, Therapy And/Or Training (Incl. AT) – Other Therapy.

Other NDIS-related work requests such as reports, review letters and travel is charged separately at a pro-rata rate and discussed prior with the participant, Support Coordinator, or Plan Manager. An invoice will be sent to the participant or their nominated plan manager following each occurrence of service.

At any time, you may end your service agreement with Clarity Therapy by letting one of our team members know no later than 2 business days before your next appointment. NDIS participants are subject to the Clarity Therapy 48 hours cancelation policy, whereby, if they are not able to give 2 working days notice for a cancelation or “no show” for an appointment, they will be charged for the full fee of the appointment. This policy ensures clients on the waitlist have equal access to appointments and is in line with the NDIS Price Arrangements and Price Limits.

If you have a complaint regarding Clarity Therapy NDIS services, this can be directed to info@claritytherapy.com.au or Clarity Therapy Director via email at mark@claritytherapy.com.au. Alternatively, complaints can be lodged directly to the NDIA on 1800 800 110.

Goods and Services Tax

GST will not apply to NDIS services as outlined in Section 28 of the GST Act. Furthermore, Clarity Therapy can confirm that: “A supply of supports under this Service Agreement is a supply of one or more reasonable and necessary supports specified in the statement of supports included, under subsection 33(2) of the National Disability Insurance Scheme Act 2013 (NDIS Act), in the Participant’s NDIS Plan currently in effect under section 37 of the NDIS Act.”

By ticking the consent box and participating in services, I am consenting that:

  • I, have read the above information and understand the reasons for the collection of my personal information and the ways in which the information may be used and disclosed and I agree to that use and disclosure.

  • I understand that it is my choice as to what information I provide and that withholding or falsifying information might act against the best interests of my assessment and therapy progress.

  • I am aware that I can access my personal and treatment information on request and if necessary.

  • I understand that if, in exceptional circumstances, access is denied for legitimate purposes, that the reasons for this and possible remedies will be made available to me.

  • I have been provided with or have been given an opportunity to obtain a copy of the Clarity Therapy privacy policy.

  • I acknowledge my file including reports, clinical progress notes, assessments, emails, letters and other documents may be subjected to Freedom of Information request, subpoena or other court action and Clarity Therapy must comply with all applicable laws.

  • I understand that Clarity Therapy may terminate services if they decided they are not able to provide me with effective counselling/therapy/coaching.

  • I understand that if I participate in Blue or Green space appointments, I am responsible for my own safety and ensuring I am not walking too far or more than I am physically able to walk. Clarity Therapy may walk with me but are in no way responsible for my general physical health during these sessions or any potential injuries I may experience.

  • I will adhere to appropriate behaviour during my Clarity Therapy appointments and understand that the below behaviours can result in the cancelation of my appointment. These behaviours include:

  • Physically or verbally aggressive behaviour including but not limited to yelling, discriminatory language, or threats of any kind.

  • Attending appointments under the influence of substances which inhibit my ability to participate in therapy.

  • I am aware Clarity Therapy is not a crisis service and will not be able to respond to contact outside of business hours.

  • I am also aware that Clarity Therapy employees are mandatory reporters and if they have concerns about my immediate safety or the safety of those around me, they are required by law to notify emergency services and thus breach confidentiality.

  • I understand that if I am not able to give 48 hours notice, I will be subject to forfeit the full cost of my appointment as per Clarity Therapy’s cancellation policy.

This policy ensures clients on the waitlist have equal access to appointments and is in line with the NDIS Price Arrangements and Price Limits.

By participating in services, I am consenting to the above listed terms.
